We know you have questions about what your pet will experience during his or her stay. We have provided the consent forms below to help answer those questions. To save time during the check-in procedure, please fill out the necessary forms as completely as possible. Surgical and anesthetic consent forms must be completed and signed digitally at AVH, at the time of check-in.
